Description
The Siouxland Community Health Foundation's 5th Annual Health Center Open will be held on July 12, 2021 at the Sioux City Country Club. This event will feature: - 18-hole scramble - Various pin prizes throughout the tournament - Sack lunch available at 11:00AM prior to the noon shotgun start - Dinner with an awards presentation after All proceeds will benefit the Siouxland Community Health Foundation. Your support of our event will provide a direct impact to the patients we serve, benefitting programs and services that help build a healthier community. Registration deadline is July 1, 2021
